Having a document (family allowance, etc.) filled out for yourself

You would like to have a document (family allowance, etc.) filled out for yourself. To do this, there are two possibilities available :

• COME PERSONALLY TO OUR OFFICES
  Come here, bringing with you your ID card, your student card and the documents to be filled in.

• SEND US A LETTER
  Please specify the nature of your request (having a special form filled out for ONEM (the National Employment Office), for Forem (the Walloon Employment Office) or for a family allowance), and attach:
  - a signed and dated copy of your own ID card
  - your student number (optional)
  - Three first-class stamps (only in Belgium) or a €5 note (Belgium and Europe)

Data protection : attestations are issued to a student personally on presentation of his student card and ID card.
A third party may only obtain attestations when in possession of a power of attorney and a copy of the student's ID card, both signed and dated by the student. For more information, click here.

Please note : The special forms for ONEM, Forem, family allowance agencies, etc. .... can also be filled out by this department.

Warning : For Office des Pensions documents, processing can only be done by post.

A fee of EUR 1.50 is charged for each attestation, certificate or form filled out and EUR 0.60 for any copy thereof (only for documents issued by the ULB)
